Enhanced Care Management and Community Supports Resources

On behalf of the California Department of Health Care Services (DHCS), Aurrera Health Group created a comprehensive set of educational resources on Enhanced Care Management (ECM) and Community Supports, two foundational components of California Advancing and Innovating Medi-Cal (CalAIM). These resources are designed to support providers that are participating, or interested in participating, in ECM and Community Supports. The resources may also benefit Medi-Cal managed care health plans, community partners and others involved in the implementation of ECM and Community Supports, as well as Medi-Cal members.

The ECM Provider Toolkit offers information about ECM and helps providers assess readiness and prepare for ECM implementation. Topics include an overview of the program and eligibility criteria, descriptions of core services, information about roles and provider requirements, and a section on becoming an ECM Provider.

The Medi-Cal Community Supports Explainer provides a high-level overview of the Community Supports initiative, including a program description, explanation of optional services, and key considerations for entities interested in becoming providers of Community Supports.

The ECM Outreach Toolkit delivers strategies and promising practices for outreach staff from Medi-Cal managed care health plans and community organizations to successfully engage and enroll eligible members.

Coordinated Care Initiative Beneficiary Toolkit and Provider Toolkit

On behalf of the California Department of Health Care Services (DHCS), Harbage Consulting developed the Cal MediConnect Beneficiary Toolkit to support beneficiaries as well as be a resource for health plans, advocates, and community organizations that engage directly with dual-eligible members.

We also created the Cal MediConnect Provider Toolkit that helps health care providers learn about the Coordinated Care Initiative, Cal Mediconnect plans, billing, and other program information.

Health Homes Program Toolkits and Provider Guide

On behalf of the California Department of Health Care Services (DHCS), Aurrera Health Group created the Health Homes Program Member Toolkit to help educate eligible Medi-Cal members about the Health Homes Program (HHP), including eligibility and enrollment, core services, and how to join.

Aurrera Health Group also created the Health Homes Program Outreach Toolkit on behalf of DCHS. This toolkit provides strategies and promising practices for outreach staff from Managed Care Plans and Community-Based Care Management Entities to successfully engage and enroll eligible members and maintain engagement once the member is enrolled in the HHP.

In addition, we created the Health Homes Program Provider Guide to help health care and community providers learn about eligibility and enrollment, core services, billing, information-sharing and other HHP information.

California MAT Expansion Project Toolkits

California continues to address the opioid epidemic by increasing access to Medication Assisted Treatment (MAT) through the state’s MAT Expansion Project.

On behalf of the California Department of Health Care Service (DHCS), Aurrera Health Group has produced and helped distribute a series of toolkits aimed at different populations that may benefit from understanding more about MAT including primary care providers, residential treatment facilities, justice system professionals, and counselors.
