REPORT: California’s Health Homes Program: Lessons Learned and Promising Practices

The Health Homes Program (HHP) is a Medi-Cal initiative that helps manage and coordinate care for enrollees with certain chronic health and/or mental health conditions who have high health care needs or who are experiencing chronic homelessness. The California Department of Health Care Services (DHCS) oversees the administration of the HHP; and 17 Medi-Cal managed care plans (MCPs) are operating the program in 12 counties. This paper is based on key lessons learned and promising practices identified throughout learning collaborative sessions.
